[Effect of preoperative long course radiotherapy on anastomotic leakage after low anterior resection for rectal cancer: a Meta-analysis].

چکیده
OBJECTIVE To assess the effect of preoperative long course radiotherapy (RT) on anastomotic leakage (AL) after low anterior resection(LAR) for rectal cancer. METHODS Medline, EMBASE, China National Knowledge Infrastructure, the Cochrane Library databases and other databases were searched for relevant studies. Correlation between preoperative long course RT and AL after LAR for rectal cancer was examined. Review Manager 5.2 software was used to pool raw data and test the heterogeneity of existing studies and to calculate the incorporated odds ratio (OR) and 95% confidence interval (95%CI). Finally, forest plots and funnel plots were created to allow for visual comparison of the results or the effect of publication bias. RESULTS A total of 881 studies were identified and 10 studies (n=7829) were eligible for the meta-analysis, including 2581 cases of preoperative RT, and 5248 cases of surgery alone without RT. There was no significant difference in anastomotic leakage rate between the two groups (OR:1.17, 95%CI:0.98-1.39, P=0.09). CONCLUSIONS Preoperative long course RT did not increase the risk of postoperative AL after LAR in patients with rectal cancer.

Does Preoperative Radio(chemo)therapy Increase Anastomotic Leakage in Rectal Cancer Surgery? A Meta-Analysis of Randomized Controlled Trials
Objective. Preoperative radio(chemo)therapy (pR(C)T) appears to increase postoperative complications of rectal cancer resection, but clinical trials have reported conflicting results. The objective of this meta-analysis was performed to assess the effects of pR(C)T on anastomotic leak after rectal cancer resection. Methods. PubMed, Embase, and the Cochrane Library were searched from January 198...
متن کاملMeta-analysis of defunctioning stoma in low anterior resection with total mesorectal excision for rectal cancer: evidence based on thirteen studies
BACKGROUND Recent studies have shown that a protective stoma can reduce morbidity in low anterior resection for rectal cancer; however, the necessity of it is still controversially discussed. METHODS We performed this meta-analysis to provide a comprehensive evaluation of the role of defunctioning stoma in low anterior resection for rectal cancer on the rates of anastomotic leakage and reoper...
متن کاملTransanal tube placement for prevention of anastomotic leakage following low anterior resection for rectal cancer: a systematic review and meta-analysis
PURPOSE Anastomotic leakage following low anterior resection (LAR) for rectal cancer is a serious complication that increases morbidity and mortality rates. Transanal tube placement may reduce postoperative anastomotic leakage rate by reducing intraluminal pressure and preventing fecal extrusion through the staple line. This meta-analysis evaluated the effectiveness of transanal tube placement ...

PURPOSE To investigate the risk factors for anastomotic leakage (AL) after anterior resection for rectal cancer with a double stapling technique. PATIENTS AND METHODS Between January 2004 and December 2011, 753 consecutive patients in Jiangsu Cancer Hospital and Research Institute diagnosed with rectal cancer and undergoing anterior resection with a double stapling technique were recruited. A...
متن کاملPredicting the risk and diminishing the consequences of anastomotic leakage after anterior resection for rectal cancer.
INTRODUCTION Anastomotic leakage is one of the most serious early complications of any intestinal anastomosis. The morbidity and mortality are high and patients may be at increased risk of cancer recurrence. In colorectal surgery the risks are particularly high following low anterior resection. Factors which increase and decrease the risks are discussed. METHODS A review of the main published...
